申請流程
遞交申請表時，請連同以下資料：Please submit the completed application form with the following: 申請學生之出生證明書(或許可在港居留證明文件，例如：港澳通行證)的影印本The photocopy of student’s Birth Certificate (or documents permitting the child toremain in Hong Kong, e.g. Permit for Proceeding to HK and Macao) 申請學生之香港身份證A photocopy of student’s Hong Kong Identity Card 申請學生之護照相片 2 張 (請在相片背面寫上姓名)Two passport-size photographs of the student (with name at the back) 父母 (或監護人) 身份證明文件影印本A photocopy of parent’s (or guardian’s) Hong Kong Identity Card(s) 英文 (或中文) 住址證明影印本A photocopy of proof of home address in English (or Chinese) 學校成績表影印本A photocopy of school report card in last year 授權書（如適用）A letter of authorisation (if applicable)
