面試成績、過去兩年的學習成績、行為、課外活動（如音樂和表演藝術等）、社會服務經驗以及申請人可能獲得的任何額外獎項或成就。

小四第三學期/下學期成績表、小五全年成績表、小六第一學期/上學期成績表(只適用於第三階段報名人士)
學生香港身份證副本
簽證副本 （只適用於非本地學生）
中文作文功課一篇(老師批改版本)
英文作文功課一篇(老師批改版本)
學生近照 (只接受證件相)

2. Original copies of submitted Awards and Certificates
獎狀及證書正本
3. Original copies of HKID card and/or study visa
身份證及/或簽證正本
4. P5 and P6 Student Handbooks with homework records

5. Recommendation Letter from Primary School (if applicable) and copies
